Assessment Tasks

Copy and paste from the following data to produce each assessment task. Write these in plain English and spell out how, when and where the task is to be carried out, under what conditions, and what resources are needed. Include guidelines about how well the candidate has to perform a task for it to be judged satisfactory.
Copy and paste from the following performance criteria to create an observation checklist for each task. When you have finished writing your assessment tool every one of these must have been addressed, preferably several times in a variety of contexts. To ensure this occurs download the assessment matrix for the unit; enter each assessment task as a column header and place check marks against each performance criteria that task addresses.

Confirm own role in the birthing process with midwife or medical practitioner. 
Communicate consistently in culturally appropriate and safe ways with women, their partners or families. 
Identify client cultural beliefs and personal preferences for their birthing from interactions and antenatal health care information and communicate these to attending midwife or medical practitioner. 
Facilitate questions and answers between women, their partners or families, and the midwife or medical practitioner, and confirm understanding. 
Facilitate culturally safe and effective communications between midwife or medical practitioner and woman throughout the birthing process. 
Work under instructions from midwife or medical practitioner and according to standard treatment protocols across the stages of labour to assist with foetal and maternal monitoring and other support activities. 
Ensure women, their partners or families understand the advice and actions of the attending midwife or medical practitioner, confirm informed consent and explain what this means. 
Provide information and encouragement to women using positive and empathetic communication. 
Identify signs of any complications during the birthing process and report immediately to attending midwife or medical practitioner. 
Monitor the emotional wellbeing of women and their partners or families during the birthing process and provide care and advice. 
Complete and accurately record routine physical checks of mothers and babies according to midwife or medical practitioner instructions. 
Support the physical comfort of mothers and babies by providing routine postpartum information and advice. 
Identify signs of immediate postpartum complications and report immediately to attending midwife or medical practitioner. 
Monitor emotional wellbeing of mothers and their partners or families immediately after birth and provide care and advice. 
Update client files accurately and according to organisational procedures.
